1. Complete and Sign Driver's Medical History (Exam Form-Page 1)

  • We provide DOT form free of cost


2.  Complete Physical Exam

  • Medical History
  • Medical Conditions
  • Height, weight, blood pressure, breathing, sight, hearing, pulse rate, range of motion
  • Urinalysis Exam


3. Complete Paper Work

  • Photo ID required
  • We will complete your paperwork
  • We will provide two copies of physical forms and laminated DOT medical card

Facts That Might Apply As You Prepare For Your DOT Physical Exam

  • INJURY

If you suffered permanent loss of use in an arm or leg you MUST bring letter from your doctor regarding how the injury happened.  You may need a SPE (Skilled Performance Exam)

  • VISION AND HEARING

If you wear eyeglasses, contacts lenses and/or hearing aids, you MUST bring them with you as youu are required to pass both a vision and hearing exam.  If there is any questions whether you will qualify you should visit your eye or hearing doctor before you examination.

  • MEDICATION

Bring a complete list of all medication with the correct doses, especially those with sleepiness as a side effect.  Must bring a letter from the prescribing provider assuring you are safe to operate a DOT registered vehicle.

  • BLOOD THINNERS

Bring a recent PT/INR letter from your doctor if you are taking Coumadin (Warfarin) that indicates you are safe to drive a DOT registered vehicle.

  • HEART TROUBLE

Must bring a letter from your heart specialist or cardiologist if you have had a heart attack or have had a stent, pacemaker or valve replacement.  Letter must outline our medical history and indicate that you are safe to drive a DOT registered vehicle.

  • STOKE

Also includes brain tumors, seizure disorder, and bleeding in the brain.  Must bring letter from your heart specialist or cardiologist describing your medical history and indicate that you are safe to drive a DOT registered vehicle.

  • HIGH BLOOD PRESSURE

Your blood sugar MUST be under 200ml/dl on the day of your exam.  Bring complete list of medication with the correct doses.

  • SLEEP APNEA OR CPAP MACHINE USE

Bring a letter from your sleep specialist or reading from you CPAP machine demonstrating proper use - at least 4 hours per night, 70% of the total nights.


Additional testing and/or information may be required by your DOT examiner.
